Responsibilities of the RN Case Manager
Complete an initial/comprehensive assessment of the patient/family to determine needs which includes a history of current and previous illness(es), as well as a physical assessment of the patient, with emphasis on the management of pain and other symptoms.
Provide advice and emotional support to patients and their family members.
Record patients' medical histories and symptoms.
Help perform diagnostic tests and analyze results
Administer treatments and medications, and help with patient follow-up and rehabilitation.
Supervise other healthcare professionals and coordinate patient care.
Comply with agency, Federal, state, and local laws, regulations, policies and procedures.
Requirements of the RN Case Manager
Registered nurse with license in the State of California--license is active & in good standing.
Evidence of current CPR certification.
Community health & a wide variety of Medical-Surgical experience preferred
