Observing, assessing, planning, implementing and evaluating nursing support for well, chronic or acutely ill individuals in a community or small facility setting
Working closely with other health professionals/providers in planning, implementing, and evaluating programs and services to address the health needs of persons served
Coordinate communications and actions of various disciplines as it relates to health and safety supports
Ensuring the ordering, reviewing, monitoring, discontinuation and, if necessary, the disposal of medications as prescribed by the physician
Conduct patient clinic and field visits
Collect patient history and conduct medical assessments for staff and consumers as needed
Plan, implement and evaluate patient care plans based on patient assessment
Fulfill clerical responsibilities such as sending/receiving patient medical records, obtaining reports, completing forms as needed, scheduling appointments, verifying insurance, maintaining patient charts, etc.
Clean and prepare instruments, equipment, and examination rooms according to department protocol
Participate in discharge planning process. Qualifications:
Education: Graduate of an accredited school of vocation nursing or accredited Diploma, Associate or Baccalaureate School of Nursing
Licensure: Current state licensure as a Registered Nurse
Skills: Current CPR accredited by American Heart Association and American Red Cross
Experience with a nursing home setting.
Experience with hospice home care.
